Information for Vendors

Insurance Requirement for Vendors, Service Providers, Contractors, and Consultants

All vendors, service providers, contractors or consultants, entering the University premises are required to show proof of adequate insurance. All departments in addition to having a signed contract approved by procurement, must obtain a Certificate of Insurance from the contractor or vendor prior to the start of any work and/or the start of a contract/agreement. The following minimum insurance standards shall apply to all Service Providers and/or Vendors performing, selling, or distributing products and services at 成人视频色情片.  If a product or service, in the opinion of the Risk Management Department and/or the General Counsel's Office, represents an unusual or exceptional risk, additional insurance requirements for that product or service may be established. 

Minimum Insurance Requirements

Statutory limits are required, and Employer's Liability Insurance with limits of not less than $1,000,000. 

Including Bodily Injury and Property Damage Liability, Independent Contractors Liability, Contractual Liability, Product Liability and Completed Operations Liability in an amount not less than $1,000,000 per occurrence, and $2,000,000 aggregate. The general liability additional insured coverage must be provided in the form of an endorsement to the contractor's insurance using ISO CG 2010 (07/04) or its equivalent, specifically naming 成人视频色情片 as additional insured.

If the services involve any work with minors, coverage shall also include Sexual Abuse and Molestation coverage in an amount no less than $5,000,000 per occurrence. 

  • The policy must cover and corresponding Certificate of Insurance must indicate:  "This insurance covers bodily injury, emotional distress or mental anguish related to any claim, cause of action or liability associated with child molestation or sexual abuse (with limits of at least $5,000,000 or higher) and shall not include an employee exclusion."
  • This policy may be in a stand-alone policy or separate coverage as part of a multi-line policy or as an endorsement to a CGL policy and must be written on an occurrence and not on a claims made or claims made and reported basis.  No exclusions in the primary policy (if it is not a stand-alone policy) may be applicable to contradict the coverage.  成人视频色情片 must be named as an additional named insured on the policy.

*All exclusions added by endorsement must be indicated and must not limit the primary coverage required.

For vendors who will drive on University property, Automobile Liability in an amount not less than $2,000,000 combined single limit for bodily injury and property damage, including owned, hired and non-owned vehicle coverage. 

$2,000,000 each claim for all professional services including, but not limited to, architects, engineers, consultants, and testing services. The coverage must be maintained during the term of the contract/agreement and at least three (3) years following its completion/termination.

For all software or technology vendors/contractors. Required limit to be determined based on type of product/service.

Additional limit of liability may evidence through an Umbrella or Excess Liability policy.  Coverage should "following form" subject to the same provisions as, the underlying policies required for General Liability and Commercial Auto Liability Insurance. The University reserves the right to request additional insurance if in its opinion a contracted product or service represents an unusual or exceptional risk; and to request, at any time, complete, certified copies of all required insurance policies, including endorsements evidencing the coverage required by these specifications.

The Contractor shall procure and maintain for the duration of the Agreement, insurance against claims for injuries to persons or damage to property which may arise from or in connection with the performance of the work hereunder by the Contractor, their agents, representatives, employees or subcontractors.

Minimum Insurance Requirements:  

Commercial General Liability: Liability in an amount not less than $2,000,000  per occurrence/$4,000,000 aggregate, on a per project basis. Insurance shall be written on ISO occurrence form CG 00 01 and shall cover liability arising from premises, operations, stop gap liability, independent contractors, products-completed operations, personal injury and advertising injury, and liability assumed under an insured contract.  The insurance shall be endorsed to provide the Aggregate per Project Endorsement ISO form CG 25 03 11 85.  There shall be no endorsement or modification of the Commercial General Liability insurance for liability arising from explosion, collapse or underground property damage. Yeshiva shall be named as an insured under the Contractor's Commercial General Liability insurance policy with respect to the work performed for the University using ISO Additional Insured endorsement CG 20 10 10 01 and Additional Insured-Completed Operations endorsement CG 20 37 10 01 or substitute endorsements providing equivalent coverage.

The Contractor's insurance coverage shall be primary insurance as respect the University.  Any insurance, self-insurance, or insurance pool coverage maintained by Yeshiva shall be excess of the Contractor's insurance and shall not contribute with it.  The Contractor's insurance shall be endorsed to state that coverage shall not be cancelled by either party, except after thirty (30) days prior written notice by certified mail, return receipt requested, has been given to Yeshiva.

No Limitation: Contractor's maintenance of insurance as required by the agreement shall not be construed to limit the liability of the Contractor to the coverage provided by such insurance, or otherwise limit the University's recourse to any remedy available at law or in equity.

Contractor shall ensure that each subcontractor of every tier obtain at a minimum the same insurance coverage and limits as stated herein for the Contractor (with the exception of Builders Risk insurance). Upon the request of the University, the Contractor shall provide evidence of such insurance.

The University is NOT responsible for any equipment or property that is lost, stolen, damage, or left behind by the contractor.  The Contractor shall assume full responsibility for all loss or damage from any cause whatsoever to any tools, Contractor's employee owned tools, machinery, equipment, or motor vehicles owned or rented by the Contractor, or the Contractor's agents, suppliers or contractors as well as to any temporary structures, scaffolding and protective fences. 

The University reserves the right to request additional insurance if in its opinion a contracted product or service represents an unusual or exceptional risk; and to request, at any time, complete, certified copies of all required insurance policies, including endorsements evidencing the coverage required by these specifications.

Such insurance must be primary and non-contributory to any other valid and collectible insurance. All polices must be written by a carrier authorized to do business in the State of New York and rated at least "A" in A.M. Bests.

Any liability coverage on a "claims made" basis should be indicated on the certificate. Renewal certificates of insurance must be provided annually until all work or services are completed.

Verification of Coverage

The service provider shall furnish 成人视频色情片 with original certificates and amendatory endorsements effective coverage required by the contract. The endorsements should be on insurance industry forms, provided those endorsements conform to the contract requirements.

All certificates and endorsements are to be received and approved by 成人视频色情片 Procurement Services and Risk Management Department before any work/service begins. Upon approval, a copy of the insurance certificate and endorsement from the service provider must be attached to the contract/agreement as an exhibit, and sent to Procurement Services for its central file.

Certificate of Insurance Request from Others

When entering into various contract and agreements with others you may be asked to provide proof of insurance. The request for proof of insurance will be provided for all 成人视频色情片 related matters by issuing a Certificate of Insurance. In order to minimize the university risk of legal liability all contracts, permits and agreements must be reviewed and approved by the Office of General Counsel and the Purchasing Department prior to requesting proof of insurance. Absolutely No Exceptions!

  1. Obtain Approval from the Office of General Counsel and the Purchasing Department
  2. Complete the "Request for Certificate of Insurance from Others" form and direct it to the Risk Management Department at riskmgmt@yu.edu.
  3. Please allow for 5 working days processing time, as certificates are process off-site. The original copy is automatically sent to the certificate holder, and a copy will be forward to the university directly. If there is an "urgent request'', please note this on the form. If all necessary supporting material is included, urgent requests will be processed within 24 working hours, ahead of the normal processing.
  4. In some cases, the party requesting proof of insurance asks to have the University insurance coverage changed to meet their requirements. The most common request is to add them as an Additional Insured to our policy. Should such change in the coverage be requested then a legible copy of the contract, agreement, or permit must be attached to the Certificate Request Form. If it is a long document, you only need to send the pages that discuss the ''insurance requirements'', ''hold harmless and/or indemnification clauses'' and the page that shows the names of the parties, time period involved limits of insurance amount, and the activity description.
  5. Please note that changes to the existing insurance coverages can result in an Additional Premium amount, which will be charge to the department requesting the change(s).
